I'm wondering if the Sense connection on the fan is damaged.
The firmware runs the fan at full speed for a few seconds at start up and reads the RPM sense line. If it detects the fan is spinning it enables it. If not, it doesn't.
So perhaps the fan isn't being detected and is disabled.
I'd check the the pins in the fan socket aren't damaged.
